CLINICAL AND IMMUNOLOGICAL APPROACH TO IMPROVING THE EFFECTIVENESS OF CARDIAC SURGERY IN PATIENTS WITH MYOCARDIAL REPERFUSION DISORDERS

This study evaluates the role of clinical and immunological monitoring in optimizing cardiac surgery outcomes complicated by myocardial reperfusion injury. A total of 122 patients undergoing surgery with cardiopulmonary bypass were analyzed. Serial measurements of IL-6, TNF-α, CRP, Fas/FasL, and Caspase-3 were performed pre- and postoperatively. Elevated cytokine and apoptosis marker levels were associated with prolonged inotropic support, low ejection fraction, and extended ICU stay. Implementation of individualized immunological correction, including anti-inflammatory and antioxidant therapy, reduced postoperative complications by 28%. The integrated clinical–immunological approach demonstrated clear efficacy in improving cardiac function and overall recovery dynamics.
