Аннотация

Нарушения проводимости сердца (блокады ножек пучка Гиса, атрио-вентрикулярные блокады, удлинение интервалов PR и QRS) широко распространены у пациентов с сердечно-сосудистыми заболеваниями и традиционно рассматриваются как вторичные или «фоновые» изменения. Однако современные клинические и популяционные исследования демонстрируют их значимую прогностическую роль в развитии хронической сердечной недостаточности, повторных ишемических событий, аритмий и сердечно-сосудистой смертности. Особое внимание уделяется блокаде левой ножки пучка Гиса и расширению комплекса QRS, которые отражают выраженную электромеханическую диссинхронию и ремоделирование миокарда. В этой связи оценка ЭКГ-показателей как доступных и воспроизводимых маркеров риска представляет собой актуальное направление современной кардиологии.
